Preoperative Diagnosis: Right-sided inguinal hernia
Post-operative Day: 2
Subjective:
- Mild incisional pain, rated as 3/10.
- Pain is well-controlled with oral analgesics.
- No nausea or vomiting.
- Appetite is fair.
- Bowel sounds present but decreased.
- Voiding without difficulty.
Objective:
- Incision site clean, dry, and intact.
- Slight redness and swelling noted.
- No signs of infection.
- Vital signs stable within normal limits.
- Abdominal distention minimal.
- Bowel sounds present in all four quadrants.
Assessment:
- Patient recovering as expected post hernia repair.
- Pain well-managed.
- Bowel function gradually returning.
- Risk for infection and incisional dehiscence.
Plan:
- Continue oral analgesics as needed.
- Encourage early ambulation and deep breathing exercises.
- Monitor incision site for signs of infection.
- Advance diet as tolerated.
- Educate patient on wound care and activity restrictions.
- Schedule follow-up appointment in one week.
Note: Patient has been discharged on postoperative instructions, including wound care, activity restrictions, and medication regimen.
Additional Notes:
- Patient has a history of hypertension, well-controlled with medication.
- Patient has no allergies.
- Patient is to return to clinic in one week for incisional assessment and removal of staples.
