Subjective:

Objective:

- Vital signs stable within normal limits.
- Abdominal distention minimal.
- Bowel sounds present in all four quadrants.
Assessment:
- Patient recovering as expected post hernia repair.

Plan:
- Continue oral analgesics as needed.
- Encourage early ambulation and deep breathing exercises.
- Monitor incision site for signs of infection.

- Schedule follow-up appointment in one week.
Note: Patient has been discharged on postoperative instructions, including wound care, activity restrictions, and medication regimen.

I am writing to introduce Ms. Mihiraa, who is recovering following a right inguinal hernia repair via laparoscopic mesh placement. She requires your continued care and assistance.
Ms. Mihiraa underwent surgery on 08/06/2024. Postoperatively, she reported mild incisional pain, rated 3/10, which is well-controlled with oral analgesics. Bowel sounds are present but decreased, and she is voiding without difficulty. Her incision site is clean, dry, and intact, with slight redness and swelling but no signs of infection.
